Before sending the questionnaire
- Confirm the questions are proportionate to the value, duration, hazards, and complexity of the proposed scope.
- Identify how completed tax, financial, insurance, and identity-related documents will be transferred and stored securely.
- Tell the subcontractor what “not applicable” means and how exceptions should be explained.
- Decide who reviews company capacity, safety, insurance, licensing, financial, and legal information.
- Set a retention period instead of keeping every response indefinitely.
Template for human review only. It does not approve a subcontractor or determine legal, tax, insurance, licensing, financial, or safety compliance.
